Amaryl tablet is indicated in following conditions-
Amaryl is indicated as an adjunct to diet and exercise to lower the blood glucose in patients with noninsulin dependent (Type II) diabetes mellitus (NIDDM) whose hyperglycaemia cannot be controlled by diet and exercise alone.

যেসব ক্ষেত্রে সন্তোষজনকভাবে নন-ইনসুলিন ডিপেন্ডেন্ট (টাইপ ২) জায়াবেটিস ম্যালাইটাস রোগীদের রক্তে গ্লুকোজের মাত্রা শুধুমাত্র খাবার এবং ব্যায়ামের মাধ্যমে নিয়ন্ত্রণ করা যায় না সেসব ক্ষেত্রে এমারিল নির্দেশিত।
খাবার ও ব্যায়ামের মাধ্যমে অথবা শুধুমাত্র এমারিল ও মেটফরমিন এককভাবে নির্দেশনায় যে সমস্ত রোগীদের রক্তে অধিক গ্লুকোজের মাত্রা নিয়ন্ত্রিত হয় না, এমন রোগীদের রক্তে গ্লুকোজ কমাতে এমারিল ও মেটফরমিন যুগ্মভাবে নির্দেশিত।
এছাড়াও খাবার ও বায়ামের মাধ্যমে অথবা শুধুমাত্র এমারিল এবং রক্তে গ্লুকোজ কমায় মুখে সেব্য এমন ওষুধ যুক্তভাবে নির্দেশনায় যে সমস্ত রোগীদের রক্তে অধিক গ্লুকোজের মাত্রা নিয়ন্ত্রিত হয় না, এমন রোগীদের ক্ষেত্রে এমারিল ও ইনসুলিন যুগ্মভাবে নির্দেশিত।
এমারিল ও ইনসুলিম যুগ্মভাবে ব্যবহারে হাইপোগ্লাইসেমিয়া বৃদ্ধি পেতে পারে।
🔹Pharmacologyফার্মাকোলজি
Glimepiride is a sulfonylurea antidiabetic agent which decreases blood glucose concentration. The primary mechanism of action of Glimepiride appears to be dependent on stimulating the release of insulin from functioning pancreatic beta cells. Glimepiride acts in concert with glucose by improving the sensitivity of beta cells to physiological glucose stimulus, resulting in insulin secretion. In addition, extrapancreatic effects like reduction of basal hepatic glucose production, increased peripheral tissue sensitivity to insulin and glucose uptake may also play role in the activity of Glimepiride. In non-fasting diabetic patients, the hypoglycaemic action of a single dose of Glimepiride persists for 24 hours.
গ্লিমেপিরাইড একটি সালফোনাইলইউরিয়া ধরনের এন্টিডায়াবেটিক ওষুধ যা রক্তে গ্লুকোজের পরিমাণ কমায়। প্রাথমিকভাবে গ্লিমেপিরাইড অগ্ন্যাশয়ের বিটা কোষগুলোকে উত্তেজিত করে ইনসুলিনের নিঃসরণ বাড়ানোর মাধ্যমে এর কার্যকারীতা প্রদর্শন করে। এটি গ্লুকোজের সমন্বয়ে বিটা কোষের সংবেদনশীলতা বৃদ্ধি করে ফলে ফিজিওলজিক গ্লুকোজের প্ররোচনায় ইনসুলিন নিঃসৃত হয়। এছাড়া অগ্ন্যাশয়-বহির্ভূত কার্যকারীতা যেমনঃ যকৃতের বেসাল গ্লুকোজ উৎপাদন কমানো এবং ইনসুলিনের প্রতি টিস্যু সংবেদনশীলতা বৃদ্ধি ও গ্লুকোজ গ্রহণ গ্লিমেপিরাইডের কার্যকারীতার উপর প্রভাব বিস্তার করে। নন ফাস্টিং ডায়াবেটিক রোগীদের ক্ষেত্রে গ্লিমেপিরাইডের একক মাত্রা ২৪ ঘন্টা ধরে রক্তে গ্লুকোজের মাত্রা নিয়ন্ত্রণ করে।
🔹Dosage & Administrationমাত্রা ও ব্যবহারবিধি
In principle, the dosage of Glimepiride is governed by the desired blood sugar level. The dosage of Glimepiride must be the lowest which is sufficient to achieve the desired metabolic control. The initial and the maintenance doses are set based on the results of regular check of glucose in blood and urine. Monitoring of glucose levels in blood and urine also serves to detect either primary or secondary failure of therapy.
Initial dose and dose titration: the usual initial dose is 1 mg once daily, if necessary, the daily dose can be increased. Any increase can be based on regular blood sugar monitoring, and should be gradual, i.e., at intervals of 1 to 2 weeks, and carried out stepwise, as follows: 1 mg -> 2 mg -> 3 mg -> 4 mg -> 6 mg.
Dose in patients with well controlled diabetes: the usual dose range in patients with well controlled diabetes is 1 to 4 mg daily.
Distribution of doses: Timing and distribution of doses are decided by the physician, in consideration of the patient's current life-style. Normally, a single daily dose is sufficient. This should be taken immediately before a substantial breakfast or if none is taken immediately before the first main meal. It is very important not to skip meals after taking the drug.
Secondary dosage adjustment: As control of diabetes improves, sensitivity to insuiin increases; therefore, Glimepiride requirement may fall as treatment proceeds. To avoid hypoglycaemia, timely dose reduction or cessation of Glimepiride therapy must be considered. A dose adjustment must also be considered whenever the patient's weight or life-styie changes, or other factors arise which cause an increased susceptibility to hypo or hyperglycaemia.
Changeover from other oral antidiabetics to Glimepiride: There is no exact dosage relationship between Glimepiride and other oral blood sugar lowering agents. When substituting Glimepiride for other such agents, the initial daily dose is 1 mg; this applies even in changeover from maximum dose of other oral blood sugar lowering agents. Any dose increase should be in accordance with guideline given above in 'initial dose and dose titration'. Consideration must be given to the potency and duration of action of the previous blood sugar lowering agent. It may be necessary to interrupt treatment to avoid additive effects which would increase the risk of hypoglycaemia.
তত্বগতভাবে, রক্তে গ্লুকোজের মাত্রা নিয়ন্ত্রণের জন্য যতটুকু গ্লিমেপিরাইড প্রয়োজন, ততটুকু এর মাত্রা হওয়া উচিত। বিপাকীয় প্রক্রিয়া প্রত্যাশিতভাবে নিয়ন্ত্রণের জন্য গ্লিমেপিরাইড এর সর্বনিম্ন প্রয়োগ রক্তে এবং প্রসাবে গ্লুকোজের মাত্রার উপর নির্ভর করে (নিয়মিত মাত্রা পর্যবেক্ষণের মাধ্যমে প্রারম্ভিক এবং নিয়ন্ত্রণ মাত্রা নির্ধারণ করা উচিত। রক্তে এবং প্রস্রাবে গ্লুকোজের মাত্রা নিয়মিত পর্যবেক্ষণের মাধ্যমে চিকিৎসার প্রাইমারী ও সেকেন্ডারী ব্যর্থতা নির্ধারণ করা যায়।
প্রারম্ভিক মাত্রা এবং মাত্রাবৃদ্ধি: সাধারণত প্রারম্ভিক মাত্রা হচ্ছে দৈনিক ১ মি.গ্রা. একবার প্রয়োজনবোধে মাত্রা বৃদ্ধি করা যেতে পারে। যে কোন প্রকার মাত্রা বৃদ্ধি রক্তে গ্লুকোজের মাত্রা নিয়মিত পর্যবেক্ষণ এর মাধ্যমে ১ থেকে ২ সপ্তাহ বিরতি দিয়ে এবং পর্যায়ক্রমিকভাবে বৃদ্ধি করা উচিত, যেমনঃ ১ মি.গ্রা.→২ মি.গ্রা. →৩ মি.গ্রা. → 8 মি.গ্রা. → ৬ মি.গ্রা. এভাবে।
ডায়াবেটিস নিয়ন্ত্রণে আছে এমন রোগীদের ক্ষেত্রে মাত্রা: যে সমস্ত রোগীর ডায়াবেটিস নিয়ন্ত্রণে আছে, তাদের ক্ষেত্রে এর মাত্রা হচ্ছে দৈনিক ১ থেকে ৪ মি.গ্রা.।
মাত্রা বণ্টন: চিকিৎসকের নির্দেশ অনুযায়ী মাত্রা ও সময়কাল নির্ধারণ করা হয়ে থাকে এবং তা রোগীর প্রাত্যহিক জীবনযাত্রার ধরণের উপর নির্ভর করে হয়ে থাকে। সাধারণভাবে দৈনিক একক মাত্রাই পর্যাপ্ত। পর্যাপ্ত পরিমাণ নাস্তা গ্রহণের পূর্বে এই ওষুধ সেবন করা উচিত। যদি সকালের নাস্তা এবং ওষুধ কোনটাই সেবন করা না হয়ে থাকে, তবে দিনের প্রথম প্রধান খাদ্য গ্রহণের ঠিক পূর্বে সেবন করা উচিত। ওষুধ গ্রহণের পর কোন ভাবেই খাদ্য গ্রহণ না করে থাকা উচিত নয়।
দ্বিতীয় মাত্রার সমন্বয়: ডায়াবেটিসের উন্নতির সাথে সাথে ইনসুলিনের প্রতি সংবেদনশীলতা বৃদ্ধি পেতে থাকে। হাইপোগ্লাইসেমিয়া পরিহার করার জন্য সময়মত গ্লিমেপিরাইড মাত্রা কমাতে অথবা চিকিৎসা বন্ধ করতে হতে পারে। যখন রোগীর ওজন অথবা জীবন ধারণের পন্থা পরিবর্তন হয়, অথবা অন্য এমন কোন কারণ থাকে, যার জন্য হাইপো অথবা হাইপার গ্লাইসেমিয়া হতে পারে, সেসব ক্ষেত্রেও মাত্রার সমন্বয় প্রয়োজন।
মুখে সেবনযোগ্য অন্য এন্টিডায়াবেটিক ওষুধ এর পরিবর্তে গ্লিমেপিরাইড সেবন: রক্তে গ্লুকোজের মাত্রা কমানোর জন্য কোন মুখে সেব্য ওষুধ এবং গ্লিমেপিরাইডের মাত্রার মধ্যে সরাসরি কোন সম্পর্ক নাই। যখন অন্য কোন ওষুধের পরিবর্তে গ্লিমেপিরাইড দেয়া হয় তখন প্রারম্ভিক মাত্রা হবে ১ মি.গ্রা.। অন্য ঔষধের সর্বোচ্চ মাত্রা থেকে পরিবর্তনের সময়ও গ্লিমেপিরাইডের প্রারম্বিক মাত্রা হবে ১ মি.গ্রা.। যে কোন রকমের মাত্রা বৃদ্ধির ক্ষেত্রে পূর্বে বর্ণিত "প্রারম্ভিক মাত্রা এবং মাত্রাবৃদ্ধি" বর্ণনা মেনে চলতে হবে। রক্তে গ্লুকোজের মাত্রা কমানোর জন্য পূর্বে ব্যবহৃত ওষুধ এর কর্মক্ষমতা ও সময়কালের উপর লক্ষ্য রাখতে হবে। আসক্তি এড়াতে মাঝে মাঝে চিকিৎসার বিরতি দিতে হবে যাতে হাইপোগ্লাইসেমিয়া না হয়।
🔹Interactionমিথস্ক্রিয়া
Based on experience with Amaryl and known interactions for other sulfonylureas, the following interactions must be considered.
In addition to insulin and other oral antidiabetic agents, drugs which may potentiate the hypoglycaemic action of Amaryl include: ACE inhibitors, aminosalicylic acid, anabolic steroids and male sex hormones, azapropazone, chloramphenicol, ciofibrate, coumarin derivatives, cyclophosphamide, disopyramide, fenfluramine, fenyramidol, fibrates, fluconazole, fluoxetine, guanethidine, ifosfamide, MAO-inhibitors, miconazole, oxpentifylline (high dose parenteral), oxyphenbutazone, para-aminosalicylic acid, phenylbutazone, probenecid, quinolones, salicylates, sulphinpyrazone, sulfonamide antibiotics, tetracyclines, tritoqualine, trofosfamide.
Drugs which may attenuate the hypoglycaemic action of Amaryl include:
এইচ ২ রিসেপ্টর এন্টাগনিস্ট, বিটা ব্লকার, ক্লোনিডিন এবং রিসারপিনের সাথে গ্রহণ করলে রক্তের গ্লুকোজের পরিমাণ কমানোর ক্ষমতা কমে অথবা বেড়ে যেতে পারে।
বিটা-ব্লকার, ক্লোনিডিন, গুয়ানেথেডিন এবং রিসারপিনের সাথে গ্রহণ করলে হাইপোগ্লাইসেমিক আক্রমণের উপসর্গসমূহ অপ্রকাশিত থাকতে পারে।
একিউট বা ক্রনিক অধিক পরিমাণ এলকোহল সেবন করা হলে এমারিল এর রক্তে গ্লুকোজের পরিমাণ কমানোর ক্ষমতা কমে অথবা বেড়ে যেতে পারে।
🔹Contraindicationsপ্রতিনির্দেশনা
Glimepiride is not suitable for the treatment of insulin dependent (type I) diabetes mellitus, or for the treatment of diabetic ketoacidosis, nor for the treatment of diabetic coma. Glimepiride must not be used in patients hypersensitive to Glimepiride, other sulfonylureas, other sulfonamides, severe hepatic dysfunction, severe impairment of renal function and dialysis patients.
টাইপ-১ ডায়াবেটিস ম্যালাইটাস রোগীদের চিকিৎসার গ্লিমেপিরাইড উপযোগী নয় অথবা ডায়াবেটিক কিটো এসিডোসিস রোগীদের ক্ষেত্রে কিংবা ডায়াবেটিক কোমা-এর চিকিৎসায় উপযোগী নয়। গ্লিমেপিরাইডের অন্যান্য সালফোনাইল ইউরিয়া, অন্যান্য সালফোনামাইডের অতি সংবেদনশীল রোগীদের ক্ষেত্রে, তীব্র যকৃত ক্রিয়ার অসমার্যকারীতায়, তীব্র রেচন ক্রিয়ার অসমকার্যকারীতায় এবং ডায়ালাইসিস রোগীদের ক্ষেত্রে প্রতিনির্দেশিত।
🔹Side Effectsপার্শ্ব প্রতিক্রিয়া
Hypoglycaemia, temporary visual impairment, nausea, vomiting, diarrhoea, abdominal pain, urticaria, fall in blood pressure.
হাইপোগ্লাইসেমিয়া, অস্থায়ী সৃষ্টি শক্তির সমস্যা, বমিবমি ভাব, বমি, ডায়রিয়া, পেটে ব্যথা, আর্টিকারিয়া, রক্তচাপ কমে যাওয়া।
🔹Pregnancy & Lactationগর্ভাবস্থায় ও স্তন্যদানকালে
Glimepiride must not be taken during pregnancy; a changeover to insulin is necessary. Patients planning a pregnancy must inform their physician, and should change over to insulin. Ingestion of Glimepiride with breast milk feeding may harm the child. Therefore, Glimepiride must not be taken by breastfeeding women. Either a changeover or complete discontinuation of breastfeeding is necessary.
গর্ভাবস্থার গ্লিমেপিরাইড সেবন করা উচিত নয়। এ সময় ইনসুলিন ব্যবহার করা উচিত। যে সমস্ত রোগী গর্ভাধারন করতে চান, পূর্বেই চিকিৎসককে জানানো প্রয়োজন এবং তখন ইনসুলিন গ্রহণ করা উচিত। স্তন্যদানকালে মাতৃদুদ্ধে গ্লিমেপিরাইড সেবন করলে নবজাতকের ক্ষতি হতে পারে সেজন্য স্তন্যদানকারী মায়েদের ক্ষেত্রে এর সেবন অনুচিত। এসময় হয় অন্য ওষুধ ব্যবহার করা উচিত অথবা স্তন্যদান বন্ধ রাখা উচিত।
🔹Precautions & Warningsসতর্কতা
in the initial weeks of treatment, the risk of hypoglycaemia may be increased and necessitates careful monitoring. If such risk present it may be necessary to adjust the dosage of Amaryl, Hypoglycaemia can almost be promptly controlled by immediate intake of carbohydrates (glucose or sugar).
চিকিৎসা শুরুর প্রথম কয়েক সপ্তাহে সতর্কতার সাথে পর্যবেক্ষণ করা উচিত, কারণ এ সময়ে হাইপোগ্লাইসেমিয়ার ঝুঁকি সবচেয়ে বেশী থাকে। যদি এ ধরনের ঝুঁকির সম্ভাবনা থাকে, তবে এমারিল এর মাত্রা সমন্বয় প্রয়োজনে করতে হবে। হাইপোগ্লাইসেমিয়া হলে তাৎক্ষণিকভাবে কার্বোহাইড্রেট (গ্লুকোেজ অথবা চিনি) গ্রহণ করার মাধ্যমে হাইপোগ্লাইসেমিয়া নিয়ন্ত্রণে আনা যায়।
